Warning! Scrapbooking can beVERY addictive!!!
I have included several pages of pregnancy....I made one page for each month of pregnancy, with a letter written to my growing baby about what I hope for her now, for her future, about questions I had about being a mom, etc. I scrapped some of these letters with ultrasound photos (be sure and get them scanned and printed onto photo safe pictures....US paper is very well known for fading very quickly....
Once she was born, I did several neat things....
1) I took a picture every month of her in the same spot, beside her same big old elephant toy. Now that she's a year old, I will scrapbook them all onto a 2 page spread.
2) I did a couple of 'our life before and after baby" pages...those are fun and easy! It's amazing how much your life will change!!
3) I also did made my husband write a few letters to her on the topic of being her daddy, and scrapped those
4) I did, of course, all the typical pics....all three of our feet together, her hands on our hands, some of his clothes, my clothes, and her clothes, all hung together in the closet, first smile, first laugh, first tooth, first trip to "________", first Christmas, sitting on Santa's lap, first time to pull up, first crawl, first bottle, first time seeing the grand-parents, etc. etc. etc.
5) I've also done lots of pages with just reflections on her and her personality already
6) Finally, I've started a book of relatives, with answers from each relative to several questions ranging from "If you could have a super power, what would it be" to "What is one piece of advice you'd give my daughter as she tackles this world?"
Each person sends me their answers and a photograph, and I'm scrapping a book of them for her, since there are many of them that she'll likely not get to meet for a long time (both my family and my husbands are spread out all across North America).
References :
You can look at my blog to see some examples:

I have started mine from the pregnancy stage. I have included pics of me while pregnant and u/s pics. I have also included some journalling about how I have been feeling and what I have been doing at different stages. Once she is born, I intend to include her hospital ID tag, copy of birth notice, etc. There are no hard and fast rules about what you should include or whether you should have separate ones, just go with whatever you think! Any items or information that mean something to you, will ultimately mean something to your child. (Especially personalised information such as when you first felt kicks, etc.) I wish my mother had of had kept information like that for me - it would have meant so much to me (especially now I am expecting.)
Good luck & have fun!
References :
26 weeks with # 1

I love scrapbooking and making books.. I am finding it hard to find the time though now that I have a baby.. so instead.. I am making a book on http://www.blub.com and the finished products look like a published paperback book. I am in the process of making one for my son's first year. They are so cute. Best Wishes. ;-)

LOL @ Ja's answer.. I saved the umbilical cord too! ;-)
References :
Mom of a 6 month old

I think you should def. include some picture of you while you were prego and the ultrasound. Also, include id tags, announcement, first picture, list of gift and visitors, first gift etc. LOL, I saved the umbilical stump ewwwwwwwwwwww.
